Adverse Event Reporting

If you have a side effect, an unexpected reaction, or a problem with a medication prescribed through Ivím Health, tell us. Reporting helps your care team support you and helps keep medications safe for everyone.

If this is an emergency, call 911 or go to the nearest emergency room.

Step 1: Contact Ivím Health.

We are your first call. Our team can help you understand what you are experiencing, connect you with your provider, and make sure your report reaches the right pharmacy.

Ivím Health
Phone: (877) 581-2210
Email: adverseevents@ivimhealth.com

Have the following ready when you contact us:

  • The name of the medication
  • The dose
  • When the medication was taken
  • What you experienced
  • When the reaction started
  • The name of the pharmacy that filled your prescription (found on your medication label)

Step 2: Contact the pharmacy that filled your prescription.

Your medication label shows which pharmacy filled your order. You can report directly to that pharmacy using the contact information below. Where no email is listed, call the pharmacy.

Step 3: Report to the FDA.

You can also report side effects and product problems directly to the U.S. Food and Drug Administration through its MedWatch program. This is optional, but the FDA encourages it.

Online: https://www.accessdata.fda.gov/scripts/medwatch/index.cfm?action=reporting.home
Phone: 1-800-FDA-1088 (1-800-332-1088)

Reporting to the FDA does not replace contacting your provider. If you have questions about your treatment, contact Ivím Health first.
